The following is a release and indemnity agreement which is required of all members and guests of SLUGS participating in SLUGS events/activities. Those without completed forms will be asked to leave and forgo participation.

I hereby state intent to participate in events/activities offered by the SLUGS, a non-profit organization.

I recognize that that any event/activity including but not limited to the hazards involving:

• traveling on the highways and/or waterways;
• participating in physical activities, such as hiking, running, swimming, volleyball;
• camping and hiking in remote sites and/or extreme environments;
• having accidents or becoming ill in remote places;
• the actions of participants, other persons; and/or forces of nature; are all taken at my own risk, and in no way will hold the SLUGS, event host, or activity facilitator responsible for any claim.

I agree to RELEASE, HOLD HARMLESSAND INDEMNIFY the SLUGS, its officers and board of directors and its members from any and all liability, claims and cause of action arising out of or in any way connected with my participation, or the participation of any minor that I am signing on behalf of, in any events/activities offered by the SLUGS.

I personally assume all risks for myself and any minor I sign on behalf of. The terms of this agreement shall serve as a release and indemnity agreement for my heirs, personal representative, and for all members of my family, including any minors. (Parents or legal guardians must sign for all persons under eighteen (18) years of age.)

I have read this release and indemnity agreement and have fully informed myself of its contents before I have signed it.
